Insurance Plans for Service Coverage:
Providers at SJPCS are independently contracted with various health insurance companies, but not all providers are credentialed with every insurer. Insurance companies may offer different plans under various names, and contracts frequently change. Please call our office to confirm whether treatment with your preferred therapist is covered by your insurance plan.

Payment Method:
All payments are made directly to your provider, not to SJPCS. Co-pays and co-insurance are due at the time of service. Providers at SJPCS accept payments via cash, check, or credit card. Please make checks payable to your provider, not to SJPCS.
